Why would a real one do that?
Somes the switches streetcar system in my city are controlled by the streetcar operators. Some streetcar operators are careless or impatient, they switched the switch before the streetcar in front of them clear the switch.

So what happens after the street car is in this crazy position? Do they have to shut the line down and back the car up to get it straight?
yes, shutting the line to back up the train and correct the switch.
If the streetcar is an articulated one, I think it would certainly derail.

Sounds like someone will have disciplinary actions.
There was a split switch accident on the subway system in Toronto many years ago. It created a major damage to the subway to the point it was written off.
