Yep, commonly (and mostly) there are only three doors per side in Plarail version, even in Plarail Advance, regardless of how many doors there were per side on the actual train.
The only Plarail standard train I remembered to have 4 doors per side is the older E321 Yamanote line, which the intermediate car features 4 openable doors per side.
However, I believe that is to simulate the actual 6-door-per-side intermediate cars instead, as shown below, unlike simulating all the doors 1-to-1 like Osaka Metro.
