The fact Fisher Price got rid of the road system and left only the motorised engines could've perhaps hinted why he's the same size and runs on rails (unlike when TOMY remade Bertie as an engine after they followed suit).
Oddly enough, there's an episode called "Race to the Rescue" where he tries to go off-rail, but he wobbles uncontrollably since he's used to track (though he does eventually regain confidence). Even then, he still runs almost strictly on rails except for maneuvers.
Regarding hi-rail fire engines in the Diabolo link: The above image is a 3D concept, but they do exist. One of numerous hi-rail conversions by German company Zweiweg for literally all types of lorries (and pickups/vans/construction equipment).
