I'm going to say 2. he crashed at the turntables with troublesome trucks, and in the same episode he got switched to a branch line mistakenly and crashed into a shed.

Season 3
21. Escape. Oliver arrives, but doesn’t crash.
22. Oliver Owns Up. Oliver is pushed into the turntable well by trucks - 1st crash.
23. Bulgy. Oliver doesn’t crash, but Bulgy does.
Season 4
20. Toad Stands By. Oliver doesn’t crash, but he inflicts GBH on S.C.Ruffey.
Season 5
18. Oliver's Find. Oliver goes down an old branch line by mistake and crashes into the shed at its end - 2nd crash.
23. Busy Going Backwards. Oliver doesn’t crash, but Toad runs out of control and ends up in a muddy pond.
Season 7
1. Emily's New Coaches. Oliver breaks down on a crossing and very nearly gets crashed into by Thomas, but just in time he’s pushed out of danger by Emily. Phew!
14. Snow Engine. Oliver has an unfortunate incident with icy rails and a snowman – 3rd crash.
Season 18
7. Duck in the Water. Duck goes for a swim, but while James and Rocky are lifting him out, Oliver crashes into them and Duck gets dropped in again. Later, Percy bangs into all three of them and Duck ends up in the drink for the third time. Let’s call all this just one incident – 4th crash.
15. Spencer's VIP. Oliver doesn’t crash, but it’s a close run thing when he ends up buffer-to-buffer with Duck.
16. Toad's Bright Idea. Oliver doesn’t crash, but he gets a nasty fright from Gator, who runs up to him in the dark with his lamp out.
